Membership Agreement

Term of Membership. Membership is for the term of one fencing year; a fencing year begins on 1 August and ends on 31 July. Membership will be pro-rated on a monthly basis. Membership will start on (mm/yyyy) ________ and continue for the remainder of the fencing year. Membership dues are charged on an annual basis.

Rate. The monthly rate is $85 for Individual Membership and $115 for Family Membership. A Family Membership includes up to 3 Fencers. The annual individual dues may be paid in either monthly ($85) or 6-month prepaid (6 x $85 = $460) or 12-month prepaid (12 x $85 = $920) increments each multi-month includes a 10% discount applied at signing and payment. Family memberships will be paid as the individual memberships are paid. Dues paid monthly are due on the 1 st of each month and late after the 5 th of each month. Membership accounts that remain unpaid for 2 months or more will incur the late fee for each month unpaid and an additional Administration Fee upon resumption of membership.

Administration Fee. A one time $25 Administration Fee will be due upon signing.

Late Fees. Late dues will incur a $25 late fee for each month late.

Escalation of Dues. Dues will not be increased during the fencing year. Gold Blade Fencing Center reserves the right to raise membership dues from fencing year to fencing year as necessary.

Club Rules. Members agree to adhere to published club rules. (encl. 1)

Medical Authorization . Members agree to sign and keep current a Medical Authorization Form. (encl. 2)

Release of Liability . Members agree to sign a Release of Liability Form. (encl. 3)

Termination. Membership may be terminated with 30 days written notice by the member or by the Club. Dues will be paid during the 30 day period.

Credit Card Authorization. Member authorizes that (type of card) _____________ number ____________________ expiration date_________ be charged if payment is not received by close of business on th 5 th of the month. This authorization continues until the end of the fencing year.
